The role

Positioned at the forefront of innovation and strategic engagement, the Activity Lead for General Practice in Aged Care (GPAC) at WAPHA embodies a role of dynamic leadership and impactful coordination. Collaborating closely with the Senior Advisor - Aged Care within the Primary Care department, you'll be tasked with orchestrating the strategic management, commissioning, and implementation of WAPHA's GPAC response and related activities. Your responsibilities include overseeing the planning, coordination, and monitoring of activities with internal and external stakeholders to ensure seamless delivery aligned with funding guidelines. You'll navigate through existing initiatives, identifying opportunities for integration, and improved resource utilisation while championing a culture of continuous improvement. 🔹 Location: WAPHA Head Office, Level 2, 1 Hood Street, Subiaco (Hybrid Role)
🔹 Salary: Band 5 $106,523.92 Plus Superannuation
🔹 Employment Type: Full-time Maximum term contract for 2 years

Key tasks and responsibilities include but not limited to:

  • Lead the strategic planning, commissioning, and implementation of activities related to general practice in aged care (GPAC) response, ensuring alignment with WAPHA's objectives.
  • Collaborate with internal and external stakeholders to ensure activities adhere to funding guidelines, governance requirements, and strategic priorities.
  • Review current activities within the context of the Health Priority Strategy, regional integration efforts, and primary care innovation, identifying opportunities for improvement and alignment.
  • Identify gaps and opportunities for integration, resource optimization, and service enhancement to improve outcomes for residents and clinicians.
  • Facilitate collaboration and engagement with program governance bodies, peak bodies, service providers, and lived experience representatives to design, implement, and evaluate WAPHA's GPAC response.
  • Provide expertise and guidance to inform planning, practice improvement, system integration, and commissioning of related activities.
  • Develop and maintain relevant plans, including activity work plans, project management plans, and communication and stakeholder engagement plans, ensuring alignment with WAPHA's goals and priorities.
  • Ensure the effective implementation and monitoring of activities, collaborating with project managers and internal stakeholders to achieve desired outcomes.
  • Commission related services in accordance with established procedures and guidelines.

About you

To be considered for this role, candidates must meet the following selection criteria:

Essential experience and education

  • Possession of a tertiary qualification in health or human services, or equivalent level of expertise.
  • Exceptional proficiency in communication, both written and verbal.
  • Demonstrated ability to operate effectively across all levels within an organisation.
  • Proven track record of leading and influencing cross-portfolio teams to achieve desired outcomes.
  • Established networks and relationships with key stakeholders, including the Department of Health, health service providers, and relevant entities.
  • Strategic mindset with comprehensive knowledge and understanding of health service delivery within the Western Australian health and social context.
  • High level of computer literacy included proficiency in Microsoft Office.
