The SLES (School Leaver Employment Support) Coordinator at Blue Tongue Pathways plays a vital role in assisting senior school students and recent school leavers within the disability industry to explore and find their pathway into the workforce. The primary focus of this position is to empower individuals by using a person-centred and strengths-based approach to create individualised capacity-building programs. The SLES Coordinator will guide and mentor support workers, ensuring they provide appropriate assistance and support to participants during their journey of skill development, work experience, and preparation for open employment.
• Collaborate with program participants, their families, and support workers to develop individualised capacity-building programs tailored to their interests, strengths, and career aspirations.
• Oversee the implementation of the SLES program, ensuring it aligns with the person-centred and strengths-based approach.
• Provide guidance and mentorship to support workers, ensuring they effectively support participants in developing essential life skills and vocation-specific skills.
• Conduct monthly meetings with Disability Support Workers
• Coordinate annual ART Conversations and monthly Check ins with Team Members.
• Maintain fortnightly rosters and communication to ensure that rosters are filled efficiently.
• Monitor the progress of participants and ensure that the program meets their needs and goals.
• Quarterly face to face check in’s with each customer and or stakeholder/decision maker to maintain quality customer experience.
• Maintain and build relationships with existing clients and key referrers such as Support Coordinators, Plan Managers and Allied Health Professions
• Provide training and ongoing support to support workers to enhance their skills and knowledge in assisting participants effectively.
• Stay up-to-date with industry best practices and developments related to disability employment and transition programs.
• Ensure accurate and up-to-date records of participant progress, achievements, and areas for improvement are maintained.
• Prepare regular progress reports for management, funding agencies, and relevant stakeholders.
• Exercise initiative in the application of established work procedures for the benefit of Service Users
• Monitoring and ensuring NDIS Quality and Safeguard Commission reporting obligations are provided and maintained.
• Raise Incident Reports and oversee any corrective actions required
