About the Role
This is a hands-on coordination role that combines people leadership, care planning, stakeholder management and problem solving. You’ll be the central link between participants, field staff and external stakeholders like guardians, clinicians and support coordinators.
You won’t just be managing from a spreadsheet. You’ll know your clients, back your team, and take calls when things hit the fan. You’ll work from home, check in with your people, and occasionally head out to visit participants face to face. It's real, it's raw, and it matters.
What You’ll Do
Support and coordinate a caseload of participants with complex needs
Lead and mentor your field team, keeping them steady and supported
Build detailed and practical support plans, then update them as things shift
Review daily notes and prepare weekly participant reports
Connect with stakeholders, manage case updates and attend case meetings
Answer participant calls and respond to the unexpected
Visit clients quarterly or as needed to keep your finger on the pulse
About You
You’re resilient, calm under pressure, and naturally empathetic
You’re a people person who can build trust fast and defuse tension
You’re organised but flexible, practical and emotionally intelligent
You might have NDIS or mental health experience, but it's not essential
You’re not afraid of the tough stuff and you know how to hold boundaries
You care deeply about your work and the people around you

Requirements
Based in SE QLD locations
Current driver’s license and own vehicle
Comfortable with flexible hours and occasional on-call (about one week per month)
Strong communication skills, tech-friendly and a self-starter
