About the role
Deliver high level Clinical Pharmacy services and leadership on a rotational basis to multiple speciality Pharmacy work units and multidisciplinary teams and to stakeholders/customers of Queensland Health in accordance with organisational goals.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
- Provide and be individually accountable for the delivery of high level Clinical Pharmacy services (including but not limited to medication history taking, medication reconciliation, development of Medication Action Plans, pharmaceutical care plans, and therapeutic drug monitoring) to multi-speciality work units and multidisciplinary teams with minimal direct supervision.
- Provide accurate, timely and appropriate complex clinical information and advice in the form of verbal and/or written information to the professional and multidisciplinary team both within and external to the facilities.
- Provide clinical practice supervision to undergraduate and post-graduate students, Intern and HP3 level Pharmacists including the monitoring and maintenance of professional clinical standards and outcomes as well as aiding in their development through formal training programs.
