Killarney Memorial Aged Care trading as K-Life is seeking a skilled and compassionate Care Coordinator to support the delivery of high-quality care and meaningful outcomes for our Home Care clients, NDIS participants, and vulnerable community members.

In this dynamic role, you’ll manage a client caseload, mentor frontline staff, and ensure services are delivered with excellence, compliance, and a person-led philosophy. You’ll also coordinate service activities including transport, social programs, and support for complex needs, while liaising with families, stakeholders, and medical professionals.

Key Responsibilities

Coordinate and manage services for Home Care, NDIS, and community support clients.

Develop and review person-led care plans that align with individual goals.

Mentor and support staff with on-the-spot coaching, guidance, and rostering relief.

Maintain accurate documentation, service agreements, and compliance records.

Liaise with families, external providers, and internal teams to deliver collaborative care.

Coordinate transport, medical bookings, community access activities, and care team communications.

Ensure services align with NDIS, aged care, and quality standards.

What We’re Looking For

✔ Cert III, Cert IV or Diploma in Disability, Aged Care, or Community Services (or similar).
✔ Minimum 1 years' experience in care coordination, community care, or NDIS-related roles.
✔ Strong understanding of NDIS, Home Care, and person-centred service delivery.
✔ Confidence in mentoring staff, conducting needs assessments, and managing complex support needs.
✔ Excellent communication and documentation skills.
✔ Current First Aid & CPR, NDIS Worker Screening Check, and Unrestricted Driver’s Licence (or willingness to obtain).
