Reporting to the Directors and working alongside a team of awesome admins, intake, Allied Health professionals and Business Development, you will bring a skill-set that challenges the status-quo, always putting the participant at the centre.
What You’ll Do
Master the Handover: Dive in and take the reins as new supports begin service with Journey Connect. At the 6-12 week mark, you will take responsibility for the supports from the Intake Officer. You’ll ensure every transition is flawless and meets top-tier quality, clinical, and NDIS compliance standards. Your eagle eye sets the stage for brilliance.
Craft Care Plans with Flair: Design tailored, ongoing Care Plans for participants—think High-Intensity Supports, In-Home Supports, and Community Access—all woven around their personal goals. Your creative spark will light up their path.
Stay Ahead of Risks: Keep a watchful eye with ongoing risk assessments, protecting participants and staff alike. Your proactive approach shines brightest in tricky, high-stakes moments. You will ensure that Restrictive Practice awareness is front and centre of risk management, maintaining dignity and rights and team safety.
Turn Dreams into Steps: Take participants’ big, bold goals and break them into bite-sized wins. You’re the genius who makes progress feel unstoppable.
Guide with Gusto: Be the go-to for our Support Workers, dishing out hands-on coaching and wisdom. Your people and clinical leadership will fuel their confidence to deliver standout daily living support and community adventures.
Train Like a Pro: Lead the charge in training our teams on fresh care plans, arming them with the skills to hit the ground running. You’ll inspire mastery in personal care, medication support, and life-changing activities.
Lead Far and Wide: Supervise support workers across multiple locations with flair, keeping quality sky-high. You’ll love the travel and thrive in supporting staff in homes and communities alike.
Shape Our Future: Jump into recruitment and help us snag top-notch support workers. Your knack for spotting empathy, grit, and goal-driven talent will build a team to be proud of.
Nurse with Impact: Oversee Community Nursing plans for high-intensity needs, flexing your clinical know-how to deliver exceptional care while guiding Support Workers to nail every detail. This will form a central part of your role over time, including managing a team of Community Nurses.
Boost Performance: Fire up our staff with clear goals, honest feedback, and a culture of growth. You’ll keep documentation spot-on and NDIS standards locked in tight.
Who You Are
Registered Nurse (essential): This means a Bachelor of Nursing (or recognised as equivalent) is your foundation with AHPRA registration (current) Your clinical skills are the backbone of this role.
Disability Support Savvy: You’ve got experience—or a hunger to dive in—and it shows.
NDIS Ninja: You know the framework inside out or are really great at learning on the go and handle compliance like a champion.
Heart for the Cause: You’re all about transforming lives for people with disabilities.
Leadership Dynamo: Your communication and inspiration skills light up any room.
Remote Team Whisperer: You manage and motivate across distances with ease.

Other essentials
You’ll need to be cleared to work by the NDIS (Yellow Card) and Working with Children approved with an annual National Police Check.
Drivers licence (not international)
Communication skills. You’ll need to be able to communicate to all ages and stages both verbally and in writing and have understanding of cultural appropriateness in our diverse demographics.
