An opportunity exists for a highly motivated and experienced Social Worker / Discharge Planner to work on a permanent part-time position working a minimum of 15 hours per week, with regular additional hours based on demand. Candidates will work across 3 days. Mondays and Fridays are required and the 3rd day is flexible.
We also have a casual position available with fixed hours for a 12-week period, followed by ongoing casual work based on demand.
It’s an ideal role for candidates seeking family friendly working hours and the work is predominantly across our rehabilitation areas.
You will provide counselling and discharge planning services to patients and be responsible for:
Providing expert clinical advice to patients, carers or other health professionals. This may include counselling, providing information regarding transfer of care services and assistance with the discharge planning process.
Developing, facilitating, implementing and evaluating appropriate discharge plans and with appropriate support.
Liaising with DVA members during their inpatient stay
Meeting the needs of patients across the surgical ward, inpatient rehabilitation and day program rehabilitation.
Essential Criteria:
Tertiary qualifications in Social Work
Demonstrated ability to provide a case management model of care
A sound understanding of patients’ individual needs to co-ordinate a safe discharge process
Experience in providing counselling
A sound knowledge of local networks, facilities and service providers which support patients on discharge
Experience working within a multidisciplinary team
High level interpersonal, written and verbal communication skills
