We are assisting a leading Asylum and refugee service who are seeking a highly organised, detail-oriented, and compassionate Case Coordinator with corrections experience to join their team for an immediate start for a ongoing contract position with the potential to go permanent. The ideal candidate will play a key role in managing and supporting individuals involved in the correctional system, working closely with a multidisciplinary team to provide comprehensive case management and ensure compliance with rehabilitation programs with experience in this area highly desired. The role aims to support clients in addressing barriers, achieving their goals, and fostering independence.

Key Duties:

• Develop and implement high-quality case plans tailored to individual client needs and goals.
• Conduct comprehensive needs assessments and facilitate referrals to support services.
• Build and maintain effective relationships with clients, families, and significant others to promote positive outcomes.
• Apply culturally appropriate client engagement principles to ensure the client's voice is central in all practices.
• Monitor and review case plans to ensure relevance and progress toward goals.
• Ensure client safety and wellbeing by completing risk assessments, safety plans, and incident reports in a timely manner.
• Maintain accurate and up-to-date records in databases, including CIRTS and funders' portals.
• Proactively communicate with internal and external stakeholders to achieve case plan objectives.
• Utilize evidence-based practices, such as the CARE and Therapeutic Crisis Intervention (TCI) models, in service delivery.

Skills and Experience:

• Tertiary qualification in Human Services or a related discipline.
• Demonstrated experience in case coordination within the Human Services sector, including conducting assessments and developing care plans.
• Proven ability to manage clients with complex physical and mental health issues.
• Experience working with individuals from culturally and linguistically diverse (CALD) backgrounds.
• Strong analytical skills and the ability to apply these to case coordination.
• Excellent written and verbal communication skills.
• Knowledge of legislative and regulatory environments relevant to the role.
• Ability to work both autonomously and as part of a multidisciplinary team.
• Current driver's licence.
