About the role

Permanent full time opportunity to demonstrate accountability at an advanced practice level for managing and coordinating ongoing care for Aboriginal and Torres Strait Islander consumers/patients with chronic/complex health care needs within COH and for the provision of specialist nursing advice, consultation, leadership, and education to interprofessional team members as relevant to the specific context of the role.

About you

Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:

  • Practice in accordance with code of ethics, professional standards and legislation affecting nursing practice by maintaining current knowledge and competence within scope of position.
  • Apply advanced nursing knowledge relevant to area of professional and clinical practice related to care of Aboriginal and Torres Strait consumer/patient with chronic/complex healthcare needs.
  • Integrate direction and priorities for quality improvement into care that establishes a quality framework which supports achievement of evidenced based care outcomes for Aboriginal and Torres Strait Islander consumers/persons.
