Position Title: Care Coordinator, Transition Care Program
Classification: Registered Nurse - YU11, Physiotherapist VA3 - VB4, Occupational Therapist VE8 - VF9, Social Worker SC12-SC24, Speech Pathologist VW1-VW4
Salary per hour (exc. super): $35.72 - $54.35 (Dependent on qualifications)
Hours per fortnight: 51.5
Employment Status: Fixed Term Part Time (until April 2026)

The Transition Care Program (TCP) Care Coordinator provides support, care planning and care coordination to clients and their carers either in their own homes and/or in a bed based setting at the conclusion of a hospital admission.
The Care Coordinator will develop collaborative relationships across the health service and community programs to provide high quality coordination, support and timely management of TCP clients. The Care Coordinator acts as the central point of contact for all involved in the care of the client and will utilise strong communication skills to liaise with the multi-disciplinary team and clients/families
