HCG have an exciting career opportunity for a Caseworker to join our team in Newcastle.
About the role:
As a Caseworker at Hunter Care Group, you will play a vital role in supporting our Support Workers who provide direct support to participants in the community. While the role primarily focuses on NDIS funded supports, you will also hold a caseload of clients supported through the Casework Support Scheme (CSS) working with the NSW Department of Communities and Justice (DCJ).
You will work closely with clinicians, family members, Support Coordinators, and, most importantly, the participants themselves, to ensure they receive high-quality, reliable support tailored to their individual needs.
Duties will include:
- Monitor and manage a caseload of NDIS participants and CSS clients, ensuring documentation, funding, and compliance are maintained.
- Support and guide Support Workers to ensure they deliver high-quality, person-centred care.
- Conduct referral meetings and assess participants' needs to develop tailored support plans.
- Create and implement participant goal plans and risk assessments.
- Build and maintain a team of suitable Support Workers for each participant by carefully matching their needs, preferences, and goals with the right workers from our support team.
- Coach and mentor Support Workers, helping them develop the skills and confidence to provide outstanding care.
- Work collaboratively with NSW Department of Communities and Justice for CSS clients, ensuring appropriate support and case management.
- Empower participants by promoting choice, independence, and quality of life.
The Successful Candidate will have:
- Certificate IV (or higher) in Community Services, Mental Health, Counselling or other relevant qualification (Bachelors Degree Desirable)
- 12 months experience supporting those with various diagnosis and co-morbidities (including experience in managing challenging behaviours)
- Ability to support participants in a person-centred way, that promotes choice, self-determination and independence.
- Excellent organisational and time management skills, and the ability to be flexible and responsive.
- Ability to work towards and meet KPI targets
- Ability to act ethically and maintain professional boundaries
- Experience or knowledge of case management in the child protection or DCJ space will be highly regarded
- Comprehensive understanding of NDIS framework and policies will be highly regarded.
Essential Criteria:
- Current Working with Children Check (or willingness to obtain)
- NDIS Worker Check Clearance (or willingness to obtain)
- NSW Drivers Licence
