Due to our continued growth, we are currently seeking an experienced Service Coordinator to join our team with an opportunity to grow within the organisation into senior roles.
About the position
The Service Coordinator is an energetic, passionate, and highly committed individual with fine attention to detail, an organised approach, and excellent administrative skills. In this position, you will be responsible for coordinating support workers and NDIS participants, liaising and developing relationships with families, other stakeholders, and onboarding new participants.
To be successful in this position, you must be someone who can effectively lead and mentor people. You will be able to implement support plans, evaluate service outcomes and match the needs of your clients with the skill sets of your support teams. You will be a self-motivated team player who is able to work collaboratively within a fast-changing environment.
Position responsibilities
- Developing and building strong service-oriented relationships with participants and their families.
- Seeking new opportunities with external stakeholders and maintaining a strong network of business referrals.
- Provide strong support and direction for disability support staff, ensuring a high level of service delivery.
- Creating support plans and risk assessments for participants that reflect the participant's needs and goals.
- Ensuring a person-centred approach when developing, implementing, and monitoring house and individual programs to meet the participants’ personal, social, educational and case planning needs.
Preferred skills
- Demonstrated understanding of and commitment to the NDIS Practice Standards and person-centred care.
- Strong administrative and management experience.
- Strong IT skills including being familiar with the Microsoft Office Suite and CRM software.
- Ability to demonstrate sound knowledge and experience in service delivery for people with disabilities and their families.
- Experience liaising with internal/external stakeholders such as support coordinators, social workers, allied health therapists, etc.
- Relevant tertiary qualifications and/or relevant experience in a similar role.
- Strong verbal and written communication skills with the ability to build and maintain good working relationships with stakeholders from a diverse range of backgrounds.
- A desire to provide friendly and professional service to the most important members of the team – our clients and our staff.
- Demonstrated problem-solving and time-management skills
Requirements
- A current South Australian Driver’s licence and a reliable vehicle.
- NDIS Worker Screening Check
- NDIS Worker Orientation Module (or willing to obtain one)
- Current Working with Children's Check
- Provide First Aid certificate (or willing to obtain one).
