About The Hospital:

North Canberra Hospital (formerly Calvary Public Hospital Bruce) and Clare Holland House are now operated by Canberra Health Services (CHS). This marks an important step as the ACT Government progresses toward the development of a new billion-dollar hospital on Canberra’s Northside.

Clare Holland House is located on the shores of Lake Burley Griffin. It is the location for Specialist Palliative Care Services in the ACT for which all patients must have a referral.

Our palliative care services are tailored to each patient and are delivered by our team of Doctors, Nurses, Physiotherapists, Social Workers and Pastoral Care professionals. We focus on supporting patients and families who are dealing with a life limiting illness. Our goal is to help patients achieve dignity, comfort and maximise the quality of life as they progress into end of life

About The Role:

As a Palliative Care Nurse Practitioner, you will play a crucial role in assessing, planning, implementing, and evaluating care for patients with serious, life-limiting illnesses. Your responsibilities will include:

• Providing comprehensive symptom management, pain relief, and psychosocial support. support to patients and their families.
• Conducting thorough assessments and developing individualised care plans to improve the quality of life for patients.
• Offering education on advanced illness, treatment options, and end-of-life care tailored to each patient’s needs.
• Collaborating with an interdisciplinary team to ensure holistic, patient-centred care.
• Maintaining accurate documentation and advocating for compassionate, dignified care at all stages of illness.

What We’re Looking For:

• Qualifications: Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA) AND endorsed to practice as a Nurse Practitioner by the Nursing and Midwifery Board of Australia ( mandatory)
• Experience: Proven experience as a Nurse Practitioner, with exceptional clinical skills and expertise. Experience in the aged care sector is highly desirable, with a deep understanding of the complexities of aging, polypharmacy, Alzheimer’s disease, other dementias, and related conditions.
• Commitment to Professional Development: A desire to engage in ongoing education and training
