Key Role Responsibilities
- Provide case management supports to youths addressing needs through a strengths-based, client-centred approach.
- Develop and nurture relationships with external services, government agencies, and community organisations to create robust support networks. Actively engage with local and specialist services to enhance resources available for clients.
- Foster a solution-focused, client-centred team culture by collaborating with colleagues, sharing knowledge, and participating in regular team meetings. Support continuous quality improvement to uphold high standards in service delivery.

Essential requirements
- Experience working in reintegration, homelessness, AOD, FDV, and/or community services.
- Demonstrated experience in case management practices and strategies to support vulnerable people.
- Demonstrated experience in developing relationships and working with people with complex needs.
- Prepared to work in multiple locations in the South West
