Support Coordinator (Core Supports and Support Coordination)

Salary expectations: $76,800 – $78,000 per year plus super

The Role

Our model of service delivery hinges on connecting clients to their local communities. Based in South Brisbane, Logan or the Gold Coast, you'll take over and manage a load of existing clients with Support Coordination to ensure our clients receive exceptional person-centred support.

Fully remote but based in the local community, this role will work from home as they are the beacon of local knowledge for our clients, connecting people to local services. Your role will include client outreach and growth, providing client support, ensuring quality and safeguarding, and fostering the development of your team.

Key responsibilities include:

Supporting clients to achieve their goals through strength-based and person-centred planning.

Coordinating NDIS packages and fostering meaningful community connections for clients.

What We’re Looking For

You are:

Passionate about making a difference, with experience in disability support or the NDIS.

A natural leader and communicator, able to inspire and build strong connections with families, stakeholders, and the broader community.

Organised and tech-savvy, with solid administration and IT skills (e.g., Microsoft Office).

Experienced in support coordination and committed to person-centred approaches.

Dedicated to fostering diversity, equity, and inclusion in everything you do.
