Responsibilities

Registered Nurse (Clinical Care Manager) - Full-Time:

- Lead and manage care teams, ensuring effective delivery of personalised client care.
- Develop, implement, and evaluate comprehensive care plans in collaboration with clients, families, and multidisciplinary teams.
- Coordinate clinical care, including medication management, wound care, continence management, and chronic illness support.
- Monitor and review care standards, ensuring compliance with healthcare regulations and best practices.
- Provide guidance, mentorship, and supervision to nursing and support staff.
- Maintain meticulous documentation and accurate, timely reporting of client conditions and care delivered.
- Conduct regular client assessments to identify needs, adjust care plans, and promote independence.
- Foster strong relationships with clients, families, community services, and healthcare professionals.
- Facilitate ongoing training and professional development opportunities for team members.

Enrolled Nurse (Casual/Part-Time):

- Monitor patient health, assess needs, and deliver optimal care.
- Administer prescribed medications and treatments, monitoring patient responses.
- Maintain precise patient records and communicate significant condition changes.
- Work within a multidisciplinary team to implement comprehensive care plans.
- Adhere to healthcare regulations and safety standards.
- Provide holistic support—physical, psychological, emotional—to clients and families.
- Participate in continuing education and professional development.

Support Worker (Casual/Part-Time):

- Provide compassionate assistance with personal care, mobility, and daily activities.
- Facilitate client community engagement and social inclusion.
- Offer emotional support and meaningful companionship.
- Support clients with meal preparation, housekeeping, and medication reminders.
- Ensure accurate client documentation and report condition changes promptly.
- Coordinate effectively with care team members for consistent quality care.
- Foster independence and assist clients in skill-building.
- Commit to professional growth through regular training opportunities.
- Valid driver's licence and willingness to travel to support clients in the community

Requirements

- Support Workers: Certificate III in Individual Support or equivalent.
- Nurses: Bachelor’s or Master's degree in Nursing (RNs), Diploma in Nursing (ENs), with experience preferably in mental health, community, home health, or aged care.
- Current AHPRA registration for nursing staff.
- Proven experience in community-based or aged care settings.
- Strong interpersonal, compassionate communication skills.
- Experience in mental health care, wound management, continence management desirable.
- Excellent documentation and record-keeping abilities.
- Valid Blue and Yellow cards.
- Current Police Check (issued within the past 12 months).
- Current First Aid and CPR certificates.
- Valid driver's licence and comprehensive vehicle insurance.
- Competence in mobile phone and digital communication technologies.
- Eligible visa to work in Australia (if applicable).
