The Role
Sanctuary is seeking a dedicated Care Coordinator to oversee the day-to-day delivery of care to residents in our client's residential aged care facility. This role is crucial in providing clinical guidance and support to care and allied health staff, ensuring the delivery of person-centred care that meets the individual clinical, social, and emotional needs of the residents.

Responsibilities:

• Provide clinical expertise and decision-making to promote the best possible nursing practice to residents.
• Supervise, mentor, and coach registered nurses in care and staff management.
• Actively participate in the delivery of care and services to residents.
• Promote and coordinate clinical documentation, including assessment, care planning, evaluation, and AN-ACC assessments.
• Review and manage incidents, ensuring timely follow-up and resolution.
• Support residents, families, and teams in coordinating case conferencing.
• Monitor clinical indicators and take proactive steps to investigate and action areas of concern.
• Ensure completion of care plan reviews and eCase documentation.

Key Skills and Requirements:

• Current registration with AHPRA as a Registered Nurse.
• A current National Police Certificate.
• Minimum of three years experience as a Registered Nurse in a senior clinical role in aged care.
• Extensive knowledge of Aged Care legislation, Aged Care Standards, AN-ACC, electronic care management systems, and OH&S legislation.
• Proven management and team leadership skills.
• Well-developed communication skills (written and verbal) and effective interpersonal skills.
• Relevant qualifications in clinical studies or management (preferred).
• Medium to high-level computer literacy, particularly with Microsoft Word, Excel, and electronic case management systems (experience with eCase is highly desirable).
• A current driver’s license.
• Up-to-date COVID-19 vaccination.
