Seeking a Care Manager (relocate) to oversee all clinical operations at a Residential Aged Care Facility in regional VIC. You will directly report to the General Manager and overseeing the clinical care in the 100+ bed home.

Duties:

• Oversee the daily clinical operations of the facility
• Support and lead the clinical team including coaching, mentoring, staff allocations and delegation, performance management and education
• Review clinical incidents, complaints and changes
• Lead compliance and accreditation preparations
• Manage person-centred care planning and evaluation for residents - including case conferencing with families and carers
• Prepare and conduct clinical governance audits
• Oversee medication management in accordance with resident needs and legislative requirements

Requirements:

• Current AHPRA as a Registered Nurse
• Previous working experience within aged care as a CCC/CM/CCM
• Proven track record in delivering quality clinical care
• Excellent clinical assessment skills and care planning capabilities
• Highly effective communication, leadership and relationship management skills
• An understanding of Resident Rights, Aged Care Accreditation Standards and Outcomes
