Position: G.P. Chronic Care Registered Nurse - Div 1, Casual 1 -2 days/week

About the Role:
Previous experience working as a Registered Nurse in a Family Medical Clinic.

We are an expanding practice that requires a dedicated Chronic Disease Management Nurse. You will work in a team which supports GP's and their patients to optimise their chronic conditions.

Essential Skills:

• Ensure all patients and visitors are received in a friendly and helpful manner, either in person or over the phone.
• Has the clinical skills to conduct detailed health assessments to improve patient health outcomes
• Identify, assess and develop chronic disease management care plans that optimises improved health outcomes
• Have a passion for managing, educating and empowering patients on preventative health care.
• Promote preventive screening
• Previous experience using Best Practice (or equivalent) clinical software.
• Maintaining demographic and clinical data in patient files often using Pen CAT.
• Maintain a high level of accuracy when recording and maintaining patient and clinical information
• Identify and provide physical and emotional patient care in your role
• Adhere to RACGP 5th Edition Standards and AGPAL Accreditation requirements

Cold Chain Management
Immunisation Certificate - competent in administering vaccinations

Essential Requirements:

• Previous Experience as a Practice Nurse in a Family Medical Clinic
• Registered Nurse - Division 1
• Current AHPRA Registered
