About the Role:
We are currently seeking an experienced Full-Time Support Coordinator | Intake Manager to join our expanding team. This is an exciting opportunity for someone looking to take ownership of both direct participant support and the intake process, ensuring smooth transitions and ongoing, effective NDIS plan management.
Following a successful qualifying period, this role will become permanent.
What you’ll be doing:
- Leading participant intake processes: Engaging with new participants, assessing eligibility, gathering essential documentation, and ensuring a seamless onboarding experience.
- Coordinating and managing NDIS plans: Supporting participants to understand and implement their NDIS plans, budget effectively, and achieve their goals.
- Managing a diverse caseload of participants with varying levels of complexity and support needs.
- Consulting with providers and stakeholders to ensure high-quality service delivery.
- Supporting high-risk participants by developing tailored strategies and connecting with external supports like Behaviour Support Practitioners, Allied Health professionals, and medical specialists.
- Evidence gathering and reporting: Assisting with reports and submissions to support appropriate funding outcomes.
- Staff training and mentoring: Providing guidance to the support team, modelling best practices, and fostering a culture of collaboration and accountability.
- Participating in continuous improvement activities, ensuring our processes and documentation meet the highest standards.
To be successful in this role, you will have:
- Proven ability to manage multiple tasks and priorities while meeting deadlines.
- Experience working with clients from culturally and linguistically diverse (CALD) backgrounds.
- Knowledge of disability-related assessments and tools (e.g., WHODAS, DSM-5, WAIS).
- Confidence in advocating for participants and holding providers accountable to deliver on participant goals.
- Exceptional communication skills and emotional intelligence when supporting families, carers, and participants.
- Strong understanding of NDIS processes and requirements.
- Financial literacy and the ability to work closely with rostering and planning teams.
- A resilient, proactive, and ethical approach to your work.
