Supported Accommodation Professional

Responsible for supporting 2 male participants in a 24/7 Supported Independent Living (SIL) setting within NDIS Practice Standards. The residence is in Capalaba. These permanent rosters will require you to commit to a fortnightly roster which will include a variety of shifts across, day shifts, sleepovers and weekends. Please only apply if you are able to commit to this variety of shifts

The men can sometimes have behaviours that can be challenging and experience with supporting people with behaviours of concern would be a real advantage.

Supported Accommodation Professionals - The Role

Myhorizon is recruiting for our SILS team (Supported Independent Living) within the Redlands area.

Join our dynamic team as a Supported Accommodation Professional.

You'll play a vital role in empowering participants to achieve community participation and independence and supporting independent living goals. Take an active role in providing direct support, guiding individuals to develop, monitor, and accomplish their aspirations within their local community or home.

You will be responsible for:

  • Empowering participants to achieve their goals by enhancing their abilities and capacities.
  • Methodically monitoring goal progress and evaluating support effectiveness, ensuring accurate documentation.
  • Delivering direct support, guidance, and supervision aligned with participants' NDIS plans.
  • Facilitate the development of specific goals outlined in participants' plans, offering continuous support.
  • Coordinate appointments, follow-up tasks, and plans to meet participants' needs effectively.
  • Providing necessary physical assistance and personalised care as specified.
  • Managing complex needs or disability-related behaviour when required, following Care Plans, Risk Assessments or Positive Behaviour Support Plans (PBSP)

Required Experience, Skills and Qualifications:

  • Equivalent Disability related qualification such as Certificate III in Individual Support (Disability) and/or willing to obtain CHC33021 Certificate III in Individual Support (Disability) within your first 12 months of commencing employment with our RTO provider, Horizon Specialised Training, RTO Code: 30008.
  • Demonstrated experience and skills in community services delivery are highly desirable.
  • Demonstrated knowledge and experience in delivering "person-centred" support within a SIL or Lifestyle environment.
  • Experience working with people who have behaviours of concern is a real advantage
  • Ability to work in a team or independently.
  • Demonstrated ability to plan, prioritise and organise your workload in a way that contributes to high levels of successful participant outcomes.
  • High level of problem-solving skills.
  • Intermediate IT skills.
  • Current NDIS Worker Screening Check or eligible to obtain before commencement (no card, no start rules apply).
  • Working with Children's Check (Blue Card) or willing to obtain prior commencement.
  • Current QLD Driver's Licence and willingness to use own vehicle with reimbursement for kilometres travelled.
  • Current Apply First Aid Certificate including CPR and/or willing to obtain prior commencement.
  • Australian Citizenship or Permanent Residency is required.
  • COVID-19 Vaccination or medical exemption highly recommended.
