Proveda is seeking a dynamic and experienced person to assist people to successfully navigate the care and support system, implement their NDIS & Home Care Plan and to enhance their ability to access the support/s they require.
About the Role
The Clinical Care Advisor (RN) will use their nursing skills to conduct clinical assessments and provide advice and guidance on safe and effective clinical care, in accordance with the customer’s needs, goals and preferences. Clinical Care includes comprehensive nursing assessment, care planning, clinical care coordination and monitoring. The Clinical Care service is predominantly outreach based and care is provided in a range of community settings including private homes and public housing and this level of experience in a community setting is a requirement for the role. The Clinical Care Advisor uses their expertise to support the Care Circles across the business in regard to safe and effective clinical care of people with complex problems. The role is responsible for liaising with General Practitioners, family members and other key stakeholders relevant to the care of the person.

About You
- Tertiary qualifications and current AHPRA registration in Medicine, Nursing,
- Demonstrated knowledge and experience in mental health, aged care, disability, chronic health care
- Demonstrated knowledge and experience in providing safe and effective clinical care tailored to the client’s needs, goals and preferences to optimise their health and well-being
- Understanding of contemporary treatment, support and community development approaches for people with high and complex care needs living in the community;
- Confidence in providing remote or virtual service;
- Detailed understanding and experience of relevant national and state legislation and health and community care service systems and standards
- High level oral and written communication, consultation, research, report writing and networking skills.
- Demonstrated experience in building customer-driven service development initiatives.
- Demonstrated ability to operate independently and professionally with limited supervision.
- Superior clinical care skills and the ability to mentor and coach colleagues and support workers to deliver safe and effective clinical care
- Capacity to interpret and work within the relevant policy guidelines.
- Comprehensive experience working in a flexible, multi-skilled team using advanced interpersonal, communication and client service skills.
- High level application skills in the use of technology-based solutions and the use of relevant computer programs
- NSW Driver’s Licence, Class C
