The opportunity

The Aged Care On-site Pharmacist program is a crucial step towards improving health outcomes for residents through integrated and supported primary health services.

In this newly established role, your primary focus will be the development and execution of an engagement plan that promotes and supports integration of the initiative. The Primary Care team is a high-impact, collaborative group passionate about strengthening the capacity of rural health and sustainable primary healthcare models.

Key accountabilities include

  • Coordinating the implementation of the Aged Care Onsite Pharmacist initiative, which will require some travel throughout our catchments.
  • Developing and delivering the engagement plan, establishing a reporting system that measures improvements to the uptake of the initiative.
  • Developing and integrating a management system that records proportion of Residential Aged Care Homes (RACH) seeking assistance to engage pharmacists.
  • Collaboration with internal teams and external stakeholders, including leading consultation with peak bodies, accredited training providers and other PHNs.
  • Support engagement between RACHs and their pharmacists and other key stakeholders, including resource development and communication strategies.

In this part-time position, you’ll enjoy flexibility to configure your work week as required to deliver on your priorities.

About you

The position will appeal to a partnership builder who is looking for a multi-faceted role that will build on their existing program delivery skills. You are a collaborator who is inspired to positively influence outcomes for Aged Care Homes, residents and their families.

Essential to the role is a degree in Health, Pharmacy or a similar field combined with relevant experience engaging with primary care and/or aged care providers.

Key capabilities include

  • Experience in the implementation of programs/services with an understanding of design and evaluation principles.
  • Demonstrable relationship and partnership development skills with the ability to influence stakeholder outcomes.
  • Strong Microsoft 365 skills with the ability to create reporting and evaluation systems using available software for ease of integration.
  • A valid driver's licence is essential.

About Murray Primary Health Network

Murray PHN (Primary Health Network) commenced on 1 July 2015 as a part of the national network of 31 PHNs across Australia, which replaced the Medicare Local program.

Murray PHN works closely with the health system to identify areas to improve, either through the better coordination and support of health services or by commissioning new services to address needs.

The catchment of Murray PHN is significant in both size and diversity. Spanning northern Victoria, and including Albury, NSW, the Murray PHN covers an area of almost 100,000 square km and it will impact health services that are accessed by more than 563,000 people.

The Australian Government has identified national objectives and priority areas for PHNs. These frame the focus of Murray PHN and will be guided by both community and clinical input to improve health outcomes.
