Cancer Care Coordinator - Haematology
This is a 48 hours per fortnight, fixed-term position ending March 2026.
Are you an experienced oncology nurse with a passion for haematology care? Mildura Base Public Hospital is seeking a skilled Haematology Cancer Care Coordinator to provide expert care and support to patients with haematological disorders. This role involves coordinating patient care, facilitating multidisciplinary collaboration, and promoting best practices in treatment and education. You will work closely with patients, families, and the Oncology team to enhance patient outcomes, improve efficiency, and ensure continuity of care.
Key Responsibilities:
- Assess and address the clinical and supportive care needs of haematology patients.
- Deliver evidence-based care and education, including clinical trial information.
- Collaborate with the clinical trials unit to recruit eligible patients.
- Act as a resource for clinical improvement and service innovation.
- Contribute to the reduction of treatment delays, readmissions, and ED presentations.
Key Selection Criteria:
- AHPRA registration as a Registered Nurse (Division 1).
- Minimum 4 years’ experience in Oncology or relevant advanced practice.
- Demonstrated understanding in the psychosocial and clinical needs of patients diagnosed with cancer.
- Ability to adapt and provide complex information and education effectively to patients’ families and carers
