About the Role:
- Provide high-quality, person-centred care to marginalised patients, ensuring their needs are met through psychosocial assessments and evidence-based interventions.
- Navigate complex case management by addressing social determinants of health and collaborating with patients, families, and carers.
- Work within a multidisciplinary team, collaborating with diverse professionals to ensure comprehensive care across multiple specialties.
- Adapt quickly to a fast-paced clinical environment, demonstrating flexibility to meet evolving patient needs and clinical challenges.
- Contribute to service improvement by supporting research, quality enhancement, and educational initiatives within the social work department.
- Take initiative for professional growth while promoting a culture of diversity, inclusion, and compassionate care within the team.
Selection Criteria:
1. Personal integrity and demonstrated commitment to the Philosophy, Mission and Values of Mary Aikenhead Ministries and St Vincent's Health Australia
2. Degree in Social Work and eligible for membership of the Australian Association of Social Workers (AASW) with evidence of clinical experience in a hospital or health related field as a practising Social Worker.
3. Demonstrated knowledge, skills and/or social work experience in mental health, drug and alcohol, domestic violence, child protection, victims of crimes, trauma, homelessness, sudden death and the coronial process.
4. Demonstrated knowledge, skills and/or experience in crisis intervention, short term solution focused intervention, grief and loss, trauma informed approaches, as well as complex assessment and discharge planning in an acute health setting.
5. Demonstrated ability to work in critical care settings as a member of diverse multidisciplinary teams in a high demand environment with competing demands.
6. Ability to contribute to research/projects/quality and education activities within social work services.
7. Well-developed time management, liaison, negotiation, computer, written and verbal communication skills.
8. Ability to participate in a seven-day service and an on call Social Work service.
