About the role
We are seeking a dedicated and compassionate Social Worker to join our dynamic Community Mental Health Team. In this vital role, you will provide high-quality mental health support and interventions as part of a multidisciplinary team. You will work closely with individuals facing complex challenges, including mental illness, substance abuse, and homelessness, offering tailored support to help them achieve their recovery goals.
Your key responsibilities will include:
- Supporting individuals with multiple complex needs, including mental health, substance use, and housing instability.
- Working as a case manager as part of the community mental health team.
- Working within a recovery framework to empower individuals, their families, carers, and community networks to make positive changes.
- Collaborating with healthcare professionals, service providers, and community organizations to create a holistic care plan that meets each person's unique needs.
- Providing a compassionate, person-cantered approach that focuses on the dignity, autonomy, and well-being of each individual.
About you
- Current Registration with the Australian Health Professionals Regulation Agency (AHPRA) or Australian Association of Social Workers.
- Extensive experience working within a Community Mental Health role as part of a multidisciplinary health care team.
- Current unrestricted NSW Drivers License to facilitate community-based support.
- Proven ability to provide high-quality consumer care, including support for families and carers of individuals with mental illness.
- Commitment to recovery-focused care, with the ability to deliver holistic mental health interventions that support individual and community well-being.
