About the role:

• Provide one-on-one support to individuals with disabilities, assisting them in accessing community resources, events, and activities.
• Develop personalized support plans in collaboration with clients and their families, focusing on individual goals and aspirations.
• Foster independence and skill development through coaching and encouragement in various community settings.
• Advocate for the rights and needs of individuals with disabilities, ensuring their voices are heard and respected within the community.
• Maintain accurate documentation of client progress and support provided, adhering to company policies and procedures.
• Collaborate effectively with colleagues, families, and external stakeholders to provide holistic support and maximize client outcomes.

What we are looking for:

• Passion for supporting individuals with disabilities and promoting inclusion in the community.
• Excellent communication and interpersonal skills, with the ability to build rapport and trust with clients.
• Empathy, patience, and a positive attitude towards challenges and learning opportunities.
• Ability to work independently and as part of a team, demonstrating flexibility and adaptability in a dynamic environment.
• Experience in disability support or a related field is preferred but not required. Training will be provided to successful candidates.
• Valid driver's license and access to a reliable vehicle (preferred).
• Either have or willingness to obtain basic First Aid & NDIS Workers Check
• Accredited or higher qualification to handle and administer medication
