The Opportunity
There is the exciting opportunity to join our dynamic and person-centred team as Youth Support Worker for the Central Burnett Youth Service, based at Gayndah. The key focus is to provide coordinated support to young people aged between 8 to 21 years who are not supported by protective factors to enable them to lead safe, healthy and active lives. This position is funded by Queensland State Government Department of Families, Seniors, Disability Services and Child Safety. This position is a full time role, however there are flexible options available to the right candidate. This position involves travel with a company car provided for business related travel.
Key Accountabilities:
The key accountabilities for this role are:
- Provide initial and ongoing assessments of young people’s needs and risks.
- Develop individualised case plans and safety plans for young people in accordance with their level of need in collaboration with support services and their families.
- Provide practical assistance and support to young people and, where appropriate, to their families to promote young people’s wellbeing, safety and reconnection to family and community.
- Provide information and appropriate referral to community resources and provide support and advocacy for young people and their families to access mainstream and specialist support services.
- Develop and facilitate networks, referral pathways and collaborative partnerships with community service providers in meeting the needs of young people.
- Ensure collection of relevant data and maintain accurate records as prescribed in a timely and efficient manner.

- Demonstrated oral and written communication and organisational skills.
- Strong computer and reporting skills.
- Are able work effectively as a member of a team and autonomously.
- Display commitment to evidence-based practice, continuous quality improvement, training, and professional development.
Selection Criteria:
- Certificate IV, Diploma or tertiary qualifications in Social Sciences or related field and/or relevant experience and competence obtained through previous service delivery or study sufficient to perform duties at this level.
- Demonstrated experience in engaging and supporting young people, particularly those who are at risk of disconnecting from family and community, experiencing disadvantage and/or displaying challenging behaviours.
- Demonstrated ability to develop networks, collaborative partnerships and referral pathways to promote the delivery of coordinated services to young people, particular to rural areas.
- Demonstrated proficiency in the use of Microsoft Office Suite applications.
- Demonstrated experience preparing accurate and comprehensive written reports and case records.
- Demonstrated commitment to personal/professional growth through supervision and participation in professional development.
- Highly developed interpersonal communication skills to communicate effectively and sensitively with a wide range of people, including Aboriginal and Torres Strait Islander Peoples and culturally and linguistically diverse communities.
A requirement prior to commencement, is the ability to obtain or currently hold the below:
- Blue Card for Working with Children
- Current NDIS Worker Screening Clearance
- Current Queensland Open Drivers Licence
- The right to work in Australia
