About the Role
You will be the face of our organisation and will have a person-centered approach to client support.
Your main responsibility will be to meet with participants of the NDIS to create and implement support plans to help achieve the NDIS participant’s goals.
This is a hybrid work from office (3 days) and from home (2 days) role with face-to-face client visits around Sydney.

Key Responsibilities
- Build and maintain strong relationships with clients and their families, ensuring their needs are met.
- Coordinate and oversee the development of personalized care plans for clients in both Disability and Aged Care Services.
- Collaborate with care teams to ensure the delivery of high-quality services.
- Act as the primary point of contact for clients, addressing inquiries, concerns, and service requests.
- Maintain accurate and up-to-date client records and documentation.
- Provide advocacy and support for clients to ensure their preferences and goals are prioritized.
- Monitor and evaluate the effectiveness of care services, making improvements where necessary.
- Assist in the recruitment and training of staff to ensure a high standard of care delivery.
- Promote Zest Care’s values and commitment to client-centered care.
About you
- Minimum Certificate 3 in Disability Services or equivalent qualification.
- Previous experience providing superior client care and engagement is essential (experience working as a NDIS Client Coordinator, Support Coordinator preferred).
- Demonstrated experience developing and maintaining client relationships.
- Sound understanding of care plan management and review.
- Knowledge of NDIS Framework and Community Services Funding Models.
- High level of computer literacy, specifically with Microsoft Office.
- Highly developed written and verbal communication skills.
- Excellent ability to connect and build rapport with multiple stakeholders.
- Experience managing complex disability or related client presentations and complexities.
- Model, mentor and monitor the work practices of staff in working within the NDIS Code of Conduct framework.
