About You:
We are seeking a passionate, hard-working Support Coordinator who performs task with a person-centered focus and a dedication to delivering outcomes in a timely manner. You will likely manage a mix of supports that are used to increase a person's capacity to maintain relationships, manage service delivery tasks, live more independently, and be included in their community.
The Role:
- Coordinate the implement supports listed in a person's plan, including informal, mainstream and community and funded supports
- Strengthen and enhance the participant’s abilities to coordinate supports and participate in the community
- Ensure mainstream services meet their obligations (i.e. Housing, education, justice, health etc.)
- Work alongside a team of allied health professionals and community networks to provide meaningful outcomes for people
- Build the capacity of an individual to achieve greater independence towards self-directing services and supports in the longer term
- Provide the NDIA with reports on outcomes and success indicators within the agreed reporting frequency
Qualifications and Skills:
- A Bachelor's degree in social work, psychology, human services, or a related field (preferred)
- Proven experience in support coordination, case management, or a related role
- Sound knowledge of disability services, government-funded programs, and community resources.
- Excellent written, interpersonal and communication skills to establish rapport with individuals, families, and service providers.
- Strong organisational skills, with the ability to prioritize tasks and manage multiple responsibilities.
- Empathy, patience, and a genuine desire to help individuals achieve their goals.
- Ability to work collaboratively within a multidisciplinary team and build positive relationships.
- Proficiency in utilising technology and computer software for record-keeping and reporting.
