Job Summary:
We are seeking an experienced Occupational Therapist to join our client’s team. This role requires rehabilitation and chronic disease management expertise, focusing on delivering personalised care to individuals facing ongoing health challenges. The ideal candidate will be compassionate, highly knowledgeable, and skilled at developing effective care plans to help clients manage their health and achieve improved outcomes.
Key Responsibilities:
- Provide direct healthcare services to patients undergoing rehabilitation and those managing chronic diseases at home.
- Assess and evaluate patient needs, designing individualised care plans based on their specific conditions and goals.
- Collaborate with interdisciplinary teams to ensure holistic and effective care delivery.
- Offer counselling and education to patients and their families on self-management strategies, lifestyle changes, and adherence to treatment plans.
- Monitor and track patient progress, adjusting care plans to address evolving health needs.
- Advocate for patient care, ensuring all services are coordinated and meet quality standards.
- Stay informed on the latest advancements in healthcare practices related to rehabilitation and chronic disease management.
- Maintain accurate and up-to-date records, adhering to all confidentiality and regulatory guidelines.
Qualifications:
- Master's or Bachelor of Occupational Therapy
- Two years of NDIS plan management experience, with a focus on rehabilitation and chronic disease management.
- Strong clinical knowledge of chronic diseases, rehabilitation methods, and patient care.
- Excellent communication and interpersonal skills, with the ability to educate and support patients and families effectively.
- Compassionate and empathetic approach to patient care.
- Ability to work collaboratively in a multidisciplinary team environment.

Pay: $90,000.00 – $120,000.00 per year
