Position Summary
This position oversees the nursing team, ensuring high-quality patient care while fostering a collaborative environment. The Team Leader will manage care plan development, supervise nursing operations, and ensure compliance with best practices and regulatory standards. Although the nursing team is nurse-led and operates with autonomy, the Team Leader provides strategic oversight to align with healthcare goals.
Collaborating with GPs and healthcare professionals, the Team Leader ensures seamless care delivery and supports the nursing team with mentorship, guidance, and professional development. This role also focuses on chronic disease management, telehealth services, and care plan management, particularly for veteran and general population patients.
The ideal candidate will have leadership experience, a strong clinical background, and a passion for patient-centered care.
Key Responsibilities
- Leadership & Team Coordination: Oversee and lead the nursing team, fostering a positive and collaborative work environment. Manage daily operations to ensure the delivery of high-quality care both in-clinic and via telehealth. Cultivate a culture of accountability, teamwork, and professional excellence.
- Care Coordination: Supervise the development and implementation of individualised care plans for patients, addressing their diverse healthcare needs, including those of the general population and veteran community. Ensure care plans are comprehensive, patient-centered, and aligned with best practices.
- Patient Care Oversight: Monitor and evaluate the quality of nursing care provided to patients, including telehealth consultations, health assessments, and chronic disease management. Support and empower both remote and in-clinic nursing teams to educate patients on medication management, health education, and lifestyle modifications, ensuring continuity and excellence in care.
- Multidisciplinary Collaboration: Act as a key liaison between the nursing team, GPs, and other healthcare professionals. Facilitate seamless communication to ensure coordinated care, timely referrals, follow-ups, and effective tracking of patient progress.
- Training & Development: Provide strategic mentorship and guidance to nursing staff, ensuring they are equipped with the knowledge, skills, and resources needed to excel. Organise and facilitate ongoing professional development opportunities to foster growth and maintain a high standard of care delivery.
- Quality Improvement & Compliance: Lead initiatives to maintain adherence to AGPAL standards, best practice guidelines, and legal regulations. Drive quality improvement measures within the practice to enhance patient outcomes and operational efficiency.
- Treatment Room Optimisation: Oversee the effective utilisation of the treatment room, ensuring optimal use of space, equipment, and staff resources. Implement streamlined workflows to enhance patient flow, minimise wait times, and deliver a superior patient experience within the clinic.
- Veteran-Centric Advocacy: Serve as a dedicated advocate for both veteran and general population patients, with a particular focus on addressing barriers to care for veterans. Provide culturally competent, trauma-informed services tailored to their unique needs. Promote emotional and psychological well-being by recognising and addressing the specific challenges veterans may encounter, ensuring they receive compassionate, holistic care alongside the broader patient community.
You:
- Qualifications: Registered Nurse with current AHPRA registration.
- Leadership Experience: Previous experience in a nursing leadership role or proven ability to manage a team.
- General Practice Experience: 2-3+ years of nursing experience in a GP clinic or similar healthcare setting (highly regarded).
- Veteran Acumen: Demonstrated understanding of veteran-specific health challenges, such as mental health, chronic conditions, or rehabilitation needs. Experience working with veteran patients or knowledge of veteran health services is highly desirable.
- Chronic Disease Management Experience (highly desirable).
- Telehealth Experience (or willingness to adapt to virtual consultations).
- Care Plan Management: Experience in developing and managing care plans, including GPMPs, TCAs, and other patient-centered care plans (highly desirable).
