Ozcare has a full-time Care Manager position available at our Townsville Community Care branch. We are looking for a compassionate individual who can help support our older generation in living independently.
Your Role
You'll be responsible for planning, managing and leading client care and support across a range of programs. You'll also play a key role in coordinating the involvement of Ozcare's multi-disciplinary teams and other services to support effective care. As a Care Manager you will:
- Manage and coordinate care services to ensure the highest standards of care
- Develop and implement care plans tailored to individual client needs
- Liaise with clients, families, and healthcare professionals to ensure comprehensive care
- Foster strong relationships with clients, stakeholders, and team members, while assisting clients in meeting their unique needs and goals
- Ensure compliance with all relevant legislation and organizational policies
- Monitor and evaluate care services to continuously improve quality and efficiency
- Monitor the budget of Home Care Packages and ensure clients understand their funding and fees

For Success you need to:
- Demonstrate experience in case management and client care coordination in a Community Aged Care setting
- Have demonstrated strong verbal, written and interpersonal communication skills to communicate and work effectively with a broad range of people
- Provide proof of immunity, or willingness to become vaccinated against, Measles, Mumps, Rubella (MMR), Varicella (Chicken Pox) & Pertussis (Whooping Cough)
- Hold a current Police Screening Certificate
- Provide a copy of your current First Aid and CPR Certificate
- Hold a current NDIS Worker Screening Check (Yellow Card) and Blue Card (Working with Children Check)
- Hold a current Queensland driver's licence
- Completed tertiary qualifications and/or relevant experience in a health-related field (e.g. Health and Human services)
