About the role

Primacy Care Australia is seeking a Service Delivery Officer to join our team in Cranbourne, VIC. In this crucial role, you will be responsible for supervising a portfolio that encompasses a team of both mental health key workers and NDIS participants ensuring they receive high-quality support and services, the role will also involve monitoring and maintaining participant support and upholding compliance related to quality and safeguarding.

What you'll be doing

• Maintaining regular and consistent communication with both participants and key workers.
• Conducting quarterly 1:1 sessions with key workers.
• Maintaining detailed Participant records to meet compliance and reporting on service delivery outcomes
• Collaborating with multidisciplinary teams to coordinate and integrate participant care
• Providing support and guidance to a team of Mental Health Key Workers
  

What we're looking for

• Relevant qualifications and experience in aged care, disability support, or a similar community services field
• Excellent interpersonal and communication skills, with the ability to build rapport and trust with clients
• Strong problem-solving and decision-making skills, with a focus on person-centred care
• A compassionate and empathetic nature, with a genuine commitment to enhancing the wellbeing of our clients
• Flexibility to work a mix of day, evening, and weekend shifts as needed
