About the Role

We are looking for a Social Worker to join our multidisciplinary team:

• To provide counselling and support to our patients and their families/carers
• To co-ordinate appropriate and timely provision of community services deemed necessary for safe discharge
• To meet with the patient and carer throughout the hospital stay to ensure that their needs and concerns have been recognised and addressed
• To complete an accurate discharge plan and document
• To establish and maintain an association with community services
• To provide education and current healthcare information to patients and carers

What We Need:

• Right to work in Australia
• Registration with the relevant professional body
• High level of communication, interpersonal and customer service skills
• Demonstrated effective time management skills and ability to prioritise competing workloads
• Ability to remain calm in stressful situations and challenging circumstances
• Ability to work both independently and as part of a team
• Ability to identify and understand customer needs and offer interventions or problem-solving strategies
• Experience in discharge planning
• Satisfactory Police Check
• Evidence of protection against specified infectious diseases, including COVID-19
