Remuneration: $37.03 - $52.83 per hour

What you'll be doing
Responsible for the provision of the high quality social work services to patients of WSLHD.
Responsible for contributing to the planning, implementation, evaluation improvement & reporting of the social work service within the Chronic and Complex team.

Essential Criteria:
- Qualifications: Degree in Social Work with eligibility for membership of the Australian Association of Social Work (AASW).
Selection Criteria:
- Demonstrated post graduate clinical experience working with clients with chronic and complex
- Demonstrated clinical experience or understanding of the needs of clients accessing the health care system.
- Demonstrated clinical experience or understanding of the needs of clients with chronic and complex health needs.
- Well-developed written, verbal and non-verbal communication skills, effective time management and ability to prioritise workload.
- Able to work as an effective team member as well as work independently, including flexibility and adaptability.
- Good computer skills, including experience using Cerner eMR or equivalent.
- Demonstrated understanding and application of quality improvement principles.
- Drivers license valid within Australia and willingness to use for work purposes.
