Our team of highly skilled professionals delivers 24/7 support across South-East Queensland, and we are seeking a Registered Nurse / Care Coordinator to be based in our Head Office in Brendale to provide clinical leadership, education, and oversight.
The Role
As an RN / Care Coordinator, you will play a vital role in assessing, planning, and coordinating care for our clients. You will work closely with our support team, clients, and their families to ensure the highest standard of care.
Key Responsibilities:
Comprehensive client assessments and development of individualised care plans.
Provide clinical oversight and guidance to disability support workers.
Deliver training and education on clinical care and best practices.
Create and review support plans, medication charts, and clinical documentation.
Oversee and manage wound care, catheter care, and continence assessments.
Review incident reports, ensuring compliance with regulatory standards.
Liaise with clients, families, and allied health professionals to ensure holistic care.
Manage and support the on-call phone service on a rotational basis.
What We’re Looking For:
Registered Nurse (AHPRA Registered) – Bachelor of Nursing qualification required.
Minimum of 3-4 years' experience in disability, community, or aged care nursing.
Strong clinical and care coordination skills.
Excellent communication and interpersonal abilities.
Passion for delivering high-quality, person-centred care.
Ability to work independently while collaborating effectively within a team.
High-level computer proficiency with the ability to quickly learn new systems.
Essential Requirements:
Current CPR & First Aid Certification
Current Blue & Yellow Card (or willingness to obtain)
Full Australian working rights
Valid Driver’s Licence & Reliable Transport
Police Check
Relevant Immunisations
