We are a private organisation and a leading national team providing in-home Aged Care to seniors with the highest standards of care. We are currently seeking outstanding and passionate Case Manager on a Full-time basis.
This desired applicant will be a key member of the Services Team providing assessment, care planning, monitoring and referrals for all clients. The Care Manager is responsible for performing in home face to face assessments and partner with clients, their carers/families to assess current situations and create case plans that enable maximum independence and positive impact for the person. Care managers also work creatively and resourcefully with participants on how they utilise their support budgets to achieve their goals.
Main responsibilities and activities will include:
1. Person centred assessment and care planning
• In consultation with clients, carers and families facilitate care assessment, care planning and review
• Develop and monitor individual plans and budgets with regular reviews/case conferencing
• Manage any identified risks within principles of Positive Approach to Challenging Behaviours and Duty of Care, Dignity of Risk
• Development and maintenance of all documentation in client management systems
• Work within legislated CDC frameworks
• Be aware of and comply with legislation and organisational requirements relating to Workplace Health and Safety

2. Client and stakeholder engagement
• Make relevant referrals, broker services and deal with suppliers as per care plan
• Develop and maintain relationships with community organisations to ensure a holistic case management approach
• Develop and maintain relationships with brokerage agencies to ensure quality services are delivered by external providers.
• Ongoing communication with clients, carers/families to ensure they are aware of rights and responsibilities to make informed choices.
Competencies -
1)Demonstrated ability to work with individuals, families and carers from a strength based approach, using high level interpersonal, verbal and written communications skills, together with negotiation liaison and conflict resolution skills
2) Demonstrated understanding and experience of current case management practice and principles such as assessment, developing and reviewing care plans, making referrals.
3) Demonstrated ability to develop quality individual plans that are consisted with principles of person centred planning within CDC frameworks.
4) Computer literacy skills and experience working with client management software systems.
5) Knowledge of local and regional networks and service providers

Requirements:
. Relevant case management experience supporting clients with high complex needs
• Successful completion or working towards a diploma/degree/post grad in Social Work, Social Welfare, Psychology or demonstrated experience
· Current First Aid Certificate (or willingness to apply)
· Current NDIS Police Check (or willingness to apply)
· Current Driver’s License and own registered working vehicle.
