Responsibilities:

• Assist participants in understanding and utilising their NDIS plans to achieve goals, and live independently, increase skills, and engage with their community and employment.
• Connect participants to allied health supports (OTs, Physiologists, Psychologists, Nurses, Dietitians), link to community, mainstream, and government services.
• Monitor, guide and assist participants exercise choice and control and coordinate supports and reviews.
• Complete necessary forms and risk assessments per policy and procedure manual and code of conduct.
• Contribute to building a stakeholder network
• Participate in meetings and share relevant training opportunities

Required:

2 years of experience in relevant case management/social work with person centred approach

Masters degree in Social Work

Drivers Licence

Full working rights

Yellow and Blue card

Desirable:

Registration with AASW or ACA or recognised body

Experience in aged care / home-care service as well as NDIS support coordination
