At Eyre and Far North Local Health Network, we are looking for passionate and caring individuals to join our team as Community Support Workers. Help empower people to live independently and make meaningful choices in their lives. By delivering compassionate, personalised care, you’ll play a vital role in our transition to a Consumer Directed Care model, ensuring consumers have control over their services and care plans.

Key Responsibilities:

Provide personal care, cleaning, meal prep, and social support.

Assist clients in maintaining their independence in their homes.

Collaborate with families, consumers, and healthcare professionals.

Complete accurate documentation and contribute to service reviews.

Offer transport, mobility assistance, and light gardening where required.

What We’re Looking For:

Certificate III in Individual Support (Ageing, Disability, or Home and Community).

Current driver’s license and access to a reliable vehicle.

Empathy, flexibility, and a proactive attitude to care.

Willingness to work weekends and on-call shifts.

Positions available:

• Ongoing Part Time working 22.8 hours per week over a 7-day roster
• Ongoing Part Time working 15.2 hours per week over a 7-day roster
