About the role

The Health on the Streets (HoTS) Program delivers a mobile assertive outreach and primary care service to vulnerable population groups including those experiencing homelessness and those sleeping rough on the Central Coast. HoTS provides medical assessments, health service navigation and non-invasive primary care treatment by the Outreach Nurse supported by the Peer Worker – HoTS to support connection to community supports, service navigation, individual advocacy and education through the provision of peer support.

The Peer Worker – HoTS will be required to undertake a range of tasks including but not limited to the following:

  • Provide peer support services with an understanding of and skills in delivering trauma informed care.
  • Provide a person-centred approach to address and support the identified individual barriers disabling a person’s health and well-being.
  • Assist in set-up of the HoTS van at scheduled hubs to enable service delivery.
  • Provide assertive outreach locating and identifying people for the HoTS program at times when hubs are not scheduled.
  • Engage with a range of service providers, community, and stakeholders to identify and engage with homeless people and rough sleepers on the Central Coast.

Key Selection Criteria

  • Certificate IV in Mental Health Peer Work and/or Community Services.
  • Demonstrated experience working in a Peer Work role.
  • Excellent verbal, written and interpersonal communication skills.
  • An understanding of and a willingness and ability to participate in the physical demands of the role. These may include working outdoors in changeable environmental conditions, working in areas inhabited by homeless persons, and dealing with challenging individuals.
  • A valid Working with Children Check (WWCC) and a Criminal Record Check (CRC) issued within the last 12 months that includes working with vulnerable groups
