WHO ARE WE?
Established in 1991, Danila Dilba Health Service (DDHS) is an Aboriginal community-controlled organisation committed to providing culturally appropriate, comprehensive primary health care services and community development within the Biluru (Aboriginal) community of the Yilli Rreung (Greater Darwin) Region in the Northern Territory.
ABOUT THE ROLE
The primary responsibility of this role is to provide practical care supports to enable Elders to remain at home living an active, self-determined and meaningful life.
It involves a mix of day respite services, personal care, domestic assistance, and social support. Client Care Plans inform what you do each day. The Aged Care Quality and Safety Commission Standards, coupled with DDHS policies, procedures, and processes guide how you do it.
The team strives to provide clients of the service with culturally safe care options both in the home, day respite centre for Elders and wider community.
PRIMARY RESPONSIBILITIES
- Provide direct, in-home care services to clients including domestic assistance, social support, and transport.
- Provide personal care services according to each client's documented Care Plans such as group social support, bathing, showering, grooming, nutrition, and oral hygiene.
- Follow procedures and practices to work to the Aged Care Quality and Safety Standards
- Establish respectful relationships with clients and their families.
- Provide day to day practical support for clients in their home and the community by:
- Reliably following a roster of scheduled appointments to deliver services to a variety of clients.
- Engage meaningfully with clients and their families while providing services as a trusted individual allowed into their personal space.
- Maintaining accurate, high quality, timely record keeping via the use of digital apps
- Identifying changes in client capabilities and proactively referring them for review when their care plan may require updating.
- Maintain client confidentiality and privacy.
- Other duties as required, consistent with skills and experience, as directed by the relevant member of DDHS.
- Participate in activities relating to incident reporting, client feedback and CQI initiatives
SELECTION CRITERIA
Essential
- Have, or partial completion of, Certificate III in Individual Support (Ageing or Disability) or equivalent
- Demonstrated understanding of the aged care system, or willingness to learn.
- Well-developed knowledge of, and contemporary experience in, working with clients with chronic health conditions and relevant management strategies.
- Knowledge of the social determinants of health (environment, economic and social inequalities) that have a profound impact on the health and well-being of Aboriginal and Torres Strait Islander people.
- Demonstrated ability to use culturally sensitive ways to ensure improved client access to DDHS and/or other health services.
- Proven teamwork, communication, and interpersonal skills with the ability to constructively negotiate outcomes with a diverse range of people and service providers.
- High level of personal resilience and flexibility with the ability to deal with challenging situations.
- Demonstrated computer skills, especially in the use of client information systems or similar.
Desirable
- Experience working in community aged care
- Qualification in Aboriginal Mental Health First Aid
- Knowledge of WH&S regulations relating to cleaning and chemicals
- Demonstrated ability to work to a plan unsupervised
- Willingness to use own vehicle in accordance DDHS Grey Fleet Policy

EMPLOYMENT IS SUBJECT TO
- A current Police Check (within 3 months) or a valid NDIS Worker Screening Check.
- A Working with Children Clearance (Ochre Card), or willingness to obtain upon commencement
- Current driver's licence
- Current first aid and CPR certificate or willingness to obtain upon commencement
- Fully vaccinated against COVID-19 or an exemption as listed on the Australian Immunisation Register (AIR) be submitted.
