About the role
We are seeking a dedicated Community Health Nurse to join our team, this role can full-time or part-time for the right applicant. You will play a vital role in providing high-quality community-based nursing services to support the health and wellbeing of our local residents.

What you'll be doing

Providing high-quality evidence based clinical care.

Triaging clients and responding to medical emergencies.

Conducting outreach services and joint case management.

Referring clients to General Practice, hospitals, support groups and community services.

Collaborating with health and community care agencies

Depending on your skill base and career interest, providing care in child and maternal health, General Practice nursing, immunisation, or health promotion.

What we're looking for

A Registered Nurse with experience in community or General Practice nursing, or transferable skills

Excellent communication and interpersonal skills to build trusting relationships with patients

Demonstrated ability to work autonomously and as part of a team

What we offer

A fortnightly Rostered Day Off (RDO) if full-time, or the option to work part-time
