Purpose of the position
INTENSIVE CARE AT HOME is looking for a clinical care coordinator for a young male spinal injury client in Sunbury. The client is in need of a 24/7 ICU nursing team at home due to ventilation and tracheostomy dependency secondary to his injury.
Our Clinical Nurse Coordinator is responsible for leading the health care team to provide safe and high quality evidence based clinical care for consumers. The Clinical Nurse Coordinator leads by example demonstrating nursing values and has well-developed professional, clinical and people management skills, achieving results through collaboration, encouragement, support and information and knowledge sharing.
Our Clinical Nurse Coordinator is responsible for the operational effectiveness of the entire team, including the day-to-day management, quality and safety for all, effective utilisation of resources, equipment and facilities and performance development and management of the clinical team.
This will be a shared job between two individuals totally 20 hours of clinical care coordinator hours. It would be essential that an individual who accepts this role can work cohesively with another individual maintaining a professional environment and supporting staff in their role.
Role of the Clinical Nurse Coordinator includes:
Responsibility 1: Management and planning of medical reviews and appointments.
Responsibility 2: Supporting clear communication to and between all staff working on the client’s program.
o This done achieved by way of education, communication book, on site – face to face.
o leading the team to implement and sustain the client’s goals of care
o Use of shadow/supernumerary shift days/nights and regular staff meetings.
o Communication channel in slack/visicase for all staff.
o A calendar of duties ensures a comprehensive plan is reached attained and maintained.
Responsibility 3: Ensuring record keeping is up to date
o This is done by way of having via a centralised data management system that includes copies of updated list are weekly submitted to the funding body.
o Utilising a standardised checklist, client care checklist, education channel for staff in slack channel. ∙
Responsibility 4: Equipment and supplies management (accident related)
o Medical Equipment testing and maintenance.
o This is done via way of a centralised equipment list, stores list and contact list.
Responsibility 5: New Staff training / familiarisation with client program
Responsibility 6: Annual competencies will be formulated by the Clinical care Coordinator Nurse that align with client care and personal needs.
o A full list of annual competencies are to be supplied to the (CCC) Nurse by MAY every year to ensure safe care of our client.
Essential qualifications
A minimum of four years CRITICAL CARE nursing experience
Current AHPRA registration Division 1
• a valid working with children check
• a police record check
• a valid driver’s license
• a valid ALS/ BLS
• NDIS worker screen
A “can-do” attitude
High level of self-awareness and ability to “read a room”
Can work independently and as part of a team
Excellent clinical judgement
Excellent non-confrontational communication skills

proven track record of managing KPI’s andthe ability to embrace them
Desirable qualifications
• Post-graduate critical care certificate or equivalent
• Experience leading a small team
• Experience in instituting and maintaining best practice.
• Experience in managing challenging environments, and de escalating tense situations.
• Experience in working within the community
• Building and maintain repour rapport with clients on a ongoing basis.
