- Classification: Registered Nurse Level 2 with salary range of: $96,705 - $101,566 per annum + Superannuation; or Health Professional Level 2 with a salary range of $91,706 - $102,613 per annum + Superannuation

About your new role
You are working as part of a transdisciplinary team which includes Intake and Assessment Workers (including both professionals and Lived Experience), Mental Health Clinical Support Coordinators, a Peer Practice Lead and a Clinical Lead. You will report to the Service Manager for the Darling Downs West Moreton and Brisbane South Medicare Mental Health Intake Assessment and Referral Services.
As Mental Health Clinical Support Coordinator, you will support the delivery of high-quality mental health intake, assessment and referral services for individuals in the community who experience mental health issues and are seeking support. A priority of the role is to connect people seamlessly to the most appropriate local service to meet their identified needs, consistent with the 'stepped care' approach. You will be responsible for ensuring continuous care co-ordination between Metro-South Addiction and Mental Health Services (MSAMHS) as well as other health service providers for consumers at times experiencing high levels of distress.
About the Program
The Medicare Mental Health phone service (MMHps formerly Head to Health Phone Service) is an exciting new service connecting the Brisbane South community to the most appropriate services that meet their identified needs. When calling the 1800 number, consumers will be greeted by mental health professionals offering mental health service navigation into and between all community mental health services. This consists of Medicare Mental Health Centres, Headspace, and other community-based mental health services, including those provided by Queensland Health, Non-Government Organisations, and private providers.
The MMHps is part of a national Medicare Mental Health brand, that enables a consistent triage and streamlined transfer or referral to the most appropriate services. A key objective of this service is local service integration to address fragmentation and offer a seamless care pathway for consumers to receive the right level of care at the right time to meet their mental health needs.
This is primarily a phone-based service, taking direct incoming referral calls through an established 1800 mental health intake number. The service uses the Initial Assessment and Referral Decision Support Tool (IAR-DST) alongside other assessments and tools, to support a nationally consistent evidence-informed initial assessment and referral process. The team is made up of staff from diverse professional and personal backgrounds including clinical, non-clinical, and lived experience. Staff work together in a transdisciplinary approach to support people to reach the appropriate service, prioritising consumer choice and agency.
The Brisbane South MMHps program is delivered in partnership with Metro South Health and Brisbane South PHN. The tripartite partnership is designed to complement - not replace or duplicate - mental health support lines and services already provided in the Brisbane South community. These existing services include crisis support lines and state-funded triage services facilitating intake into tertiary services.
The MMHps is co-located with 1300 MH CALL, a confidential mental health triage service that delivers a single point-of-access to Metro South Addiction and Mental Health Service (MSAMHS).
The MMHps operates from 8:30am to 5:00pm Monday to Friday. Opening hours may be adjusted over time in response to community needs and service demands.

What you will bring to the role
- Proven experience at working within multidisciplinary teams within the area of mental health service delivery.
- Demonstrated skill in the application of recovery-based approaches in a clinical setting.
- Experience working as a member of a multi-disciplinary (or trans-disciplinary) team with the ability to inspire and facilitate teams toward shared objectives and creating environment that encourages innovation and excellence.
- Demonstrated experience in preparation of reports with recommendations.
- Demonstrated ability to form and nurture partnerships with government and non-government mental health, emergency, and community service providers.
- Formal tertiary qualifications in Nursing, Psychology, Social Work or Occupational Therapy
- Current full registration as Registered Nurse with current annual practicing certificate from the Australian Health Practitioner Regulation Agency (AHPRA) essential or
- Current full registration as a Psychologist, Occupational Therapist or Registered Nurse with annual practicing certificate from the Australian Health Practitioner Regulation Agency (AHPRA) essential, or full registration as a Social Worker with the Australian Association of Social Workers (AASW) or can evidence membership eligibility AASW.
- Compliant with Queensland Health's workplace health and safety requirements, including Requirements for Workers in Healthcare Settings.
