Your Opportunity

As a Case Manager with Navig8, you'll play a pivotal role in helping young people move from state care to independent living, giving them the tools and confidence to build a future on their own terms.
Working across Bunbury and the wider South West and Great Southern regions, you'll be part of a passionate team making a real difference. Your Key Responsibilities
- Respond to referrals to the service from internal and external support services and conduct over the phone and formal face-to-face assessments of suitability for support
- Induct clients into the service including the program requirements, their rights, and responsibilities
- Provide structured, person-centred case management, ensuring young people stay on track toward independent living.
- Run regular case management sessions, tailoring support to each individual's cultural strengths and personal needs.
- Assist clients in developing life skills, securing stable housing, and linking with education, employment, and community resources.
- Support young people in transitioning to independence, helping them build self-reliance and confidence.
- Provide outreach services, meeting clients in environments where they feel safe and comfortable.
- Maintain detailed case records, ensuring privacy, accuracy, and compliance with policies.
- Build strong networks with community services, government agencies, and Indigenous organisations to enhance client outcomes.
Requirements for Success
With a significant background in child, youth & family sector, you will be confident in your case management processes, ensure the right to self-determination drives your practice, while promoting the wellbeing of young people. Other requirements include:
- A relevant degree with experience in case management, youth work, social work, or community services.
- Strong case management skills, with experience supporting young people with complex needs.
- Ability to build trust and mentor young people, offering structured guidance while maintaining clear professional boundaries.
- Ability to work in a culturally safe, trauma-informed manner, with strong knowledge of Aboriginal culture.
- Solid problem-solving and decision-making skills, with a natural ability to take initiative.
- Strong written and verbal communication, including report writing and case documentation.
- Excellent time management, able to handle competing priorities in a fast-moving environment.
- High-level computer literacy, including proficiency in Microsoft Office and case management systems.
- Additional WA CPFS Child Protection Screening Checks
- Must have a Full driver licence with no restrictions.
Before starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.
