About Centrecare
Centrecare is a not-for-profit organisation with over 300 employees and is committed to delivering quality professional counselling, support, accommodation, family violence support, mediation and justice programs in over 11 locations across the Perth Metro, Goldfields and Bunbury region.
About the Service
The Homelessness Services Connector is Phase Three of the expansion of Entrypoint Perth. The service will support the Department of Communities Find My Way portal. The Find My Way portal is an online tool that can be utilised by members of the Western Australian public to access information on specialist homelessness services (SHS) in Western Australia. For people experiencing or at risk of primary homelessness, there will be an option via the portal to connect online with a Homelessness Service Connector and we are looking to fill three positions.
About the role
As one of our Homelessness Service Connector, you will work primarily within an online platform to connect people who are experiencing or at risk of primary homelessness with specialist homelessness services by:
- Triaging clients and making appropriate referrals to available services and/or accommodation.
- Liaising with specialist homelessness services to progress referrals.
- Communicating to service users the outcome of referrals via an online tool.
A few specifics
- Actively network, foster and establish linkages with specialist homelessness services.
- Assist in providing up-to-date information on a range of accommodation options and support service resources in the metropolitan area.
- Enter all required data and information Specialist Homelessness Information Platform.
- Make referrals to available services and/or accommodation.
- Work collaboratively with specialist homelessness services and clients to provide a range of referrals to services.
What we are looking for
Well-rounded and experienced workers with appropriate qualifications and demonstrated skills and experience of working to support clients with issues relating to homelessness and Family and Domestic Violence.
To be successful, you will need:
- Recognised qualifications, Diploma in Community Services, Social Work or related disciplines.
- Experience working with homelessness services and supporting clients facing FDV.
- Highly proficient in computer literacy.
- Ability to work with a broad cross section of people.
- Valid driver’s licence.
- National Police Certificate and Working with Children Card
