About the Role
At WVPHN, we strive for continuous quality improvement that enhances care, supports evidence-based practice, creates efficiencies, and develops capacity and capabilities within primary care.
As a part of the Healthy Ageing portfolio, the General Practitioner (GP) Matching Program connects GPs with residential aged care homes to improve access for residents to primary care services. The Project Officer - Healthy Ageing position supports the coordination and delivery of this program and will work with relevant internal WVPHN team members and external stakeholders to deliver on this and other Healthy Ageing projects.
This is a hybrid role: working from anywhere in the western Victoria region, with frequent travel and face-to-face meetings required for stakeholder engagement plus training/team days.
Duties include:
Support the Healthy Ageing team on the development, implementation, monitoring, delivery, and evaluation of all aspects of the projects including reporting
Support the documentation of project risks, assumptions, issues, dependency, decisions and document delegations for action
Support the Healthy Ageing team to manage contracts with commissioned providers within the Healthy Ageing program
Work collaboratively within the Healthy Ageing team to support participating RACHs within our region
Provide support on identifying and documenting key stakeholder interest in the projects, including maintaining CRM
Key Selection Criteria
Experience in supporting or delivering projects
Excellent administrative skills, including writing and maintaining project documentation
High level organisational and time management skills
Strong interpersonal and communication skills (oral and written), including the ability to engage with a diverse range of stakeholders, using a customer service approach.

A current Victorian driver’s licence
