Duties:
A mix of activities and duties involving helping patients achieve their goals.
Collaborating with, and supporting a multidisciplinary team of health professionals to provide comprehensive health care towards the health of every patient.
Undertake health screening and health promotional activities.
Conduct comprehensive health assessments of patients.
Coordinate and participate in case conferences with a team of other health professionals and providers.
Assess and develop health care plans that advise health care interventions and activities in collaboration with the GPs and other health care professionals.
Participate in and assist with chronic disease management activities such as preparing and reviewing a management plan, team care arrangements and providing education about self-help strategies.
Coordinate practice recall systems in order to promote evidence based high quality health care for patients.
Provide care in the general practice environment as well as in homes and other community settings where needed.
Assist with minor procedures undertaken by the general practitioner.
Participate in and coordinate team care arrangements in collaboration with GPS and other health professionals.
Provide timely and accurate information and health education to patients.
Conduct assessments and procedures including injections, vaccinations, pathology collection/ phlebotomy, wound care, removal of sutures, blood pressure checks, ECG and other relevant procedures where needed.
Desirable skills:
Previous experience as a General Practice Nurse or Chronic Disease Nurse desirable but not essential.

Ability to communicate effectively with a variety of people
Maintain confidentiality
Prioritize and plan work to meet deadlines
Deal with problems or queries in a calm and professional manner
A mature and friendly approach when dealing with patients, families and colleagues.
Ability to undertake training and learn new skills if required.
