Launnie Care Services is a NDIS provider committed to delivering high-quality, person-centered supports across Tasmania. We work alongside individuals with diverse needs, helping them build life skills, access services, and engage with their local communities. Launnie Care Services have established in July 2022 and since then provide support to people with Disability and making a difference in their live by providing consistency.
About the Role
As a Community Worker, you’ll be working directly with individuals, families, and groups to enhance community inclusion, social participation, and access to essential services. You’ll identify needs, coordinate support, and empower clients to achieve their goals through tailored programs and advocacy.
Description
Provide support, information and advice to clients on emotional, financial, recreational, health, housing and other social welfare matters, and evaluate and coordinate the services of welfare and community service agencies.
Tasks
Assessing clients’ needs and planning, developing and implementing educational, training and support programs
Interviewing clients and assessing the nature and extent of difficulties
Monitoring and reporting on the progress of clients
Referring clients to agencies that can provide additional help
Assessing community need and resources for health, welfare, housing, employment, training and other facilities and services
Liaising with community groups, welfare agencies, government bodies and private businesses about community issues and promoting awareness of community resources and services
Supporting families and providing education and care for children and disabled persons in adult service units, group housing and government institutions
Supervising the support worker who provide support to Probation and parole participants.
Assisting young people to solve social, emotional and financial problems
Preparing submissions for funding and resources, and reports to government bodies and other agencies.

What We’re Looking For:
Qualification in Community Services, Social Work, or a related field (Diploma or higher)
Previous experience in a community or human services role
Excellent communication and problem-solving skills
Strong understanding of community development and social justice principles
Valid Driver’s License and reliable transport
NDIS Worker Screening Check and Working with Vulnerable People Check (or willing to obtain)
Please note: at least three years of relevant experience may be a substitute for the formal qualifications listed above. In some instances, relevant experience and/or on-the-job training may be required in addition to the formal qualification.
